Frequently asked questions

What do you need in a referral?

The client’s name and phone number, the service requested, the funding type and the reason for referral. For Medicare, a signed and dated GP referral letter under a GP Chronic Condition Management Plan. For DVA, a D904 or referral letter stating the condition to be treated.

Do you accept the new Medicare referral letters?

Yes. We accept referral letters written under a GP Chronic Condition Management Plan. The letter doesn’t need to name our practice.

Will I receive reports?

Yes. With the client’s consent, we send reports to the referrer. For Medicare we report after the first and last service, and for DVA at the end of each treatment cycle.

Do you see clients at home or at work?

Yes. We offer home and school visits across northern Adelaide, as well as appointments at our Salisbury Plain clinic.

Are you NDIS registered?

Not at this stage. We work with self-managed and plan-managed NDIS participants.
