Exercise physiology under a GP Chronic Condition Management Plan is bulk billed, with no gap. If you have a chronic condition, your GP can refer you to our Accredited Exercise Physiologists in Salisbury Plain under Medicare.

How Medicare works for exercise physiology

Since 1 July 2025, GP Chronic Condition Management Plans have replaced the older GP Management Plans and Team Care Arrangements (previously known as Chronic Disease Management or Enhanced Primary Care plans).

  1. Book an appointment with your GP. Let them know you’re seeking support for a chronic condition or ongoing health issue.
  2. If eligible, your GP will prepare a GP Chronic Condition Management Plan and write you a referral letter to exercise physiology.
  3. You may be entitled to up to five Medicare-subsidised sessions per calendar year, shared across all eligible allied health services (up to 10 for Aboriginal and Torres Strait Islander people).
  4. We bulk bill your exercise physiology sessions under the plan, so there’s no gap to pay.

Referral letters are valid for the period your GP states or, if no period is stated, for 18 months from your first session. If you had a GP Management Plan and Team Care Arrangement in place before 1 July 2025, you can keep using it until 30 June 2027.

Who is eligible?

You may be eligible if you have a chronic medical condition that has been, or is likely to be, present for six months or more, and your GP has prepared a GP Chronic Condition Management Plan for you. Your GP decides whether a referral to exercise physiology is right for you.

Conditions we work with

Plus many more. If you’re not sure, ask your GP or give us a call.

What your program looks like

Your Exercise Physiologist will assess your health, fitness and goals, then develop a personalised program to help you manage your condition, focusing on strength, mobility, balance, cardiovascular health and overall function. Sessions can be at our all abilities, wheelchair accessible rehabilitation facility, in the pool at the Parafield Gardens Swim School, or at home. With your consent, we send a written report to your GP after your first and last sessions.

After your Medicare sessions

Once you’ve used your Medicare sessions for the year, you can continue with private health insurance (most funds include Exercise Physiology under extras cover) or as a self-funded client. Please call us to discuss fees. You can’t claim Medicare and private health for the same session.

Frequently asked questions

Is there a gap for Medicare exercise physiology?

No. Exercise physiology under a GP Chronic Condition Management Plan is bulk billed, with no gap.

How many Medicare sessions do I get?

Up to five Medicare-subsidised allied health sessions per calendar year, shared across all allied health services, or up to 10 for Aboriginal and Torres Strait Islander people. Unused sessions don’t carry over to the next year.

My GP gave me a GP Management Plan before July 2025. Is it still valid?

Yes. If you had a GP Management Plan and Team Care Arrangement in place before 1 July 2025, you can continue to use it until 30 June 2027.

Do I need a new referral each year?

Not necessarily. Referral letters are valid for the period your GP states or, if none is stated, for 18 months from your first session. Your sessions reset each calendar year.

Can I use private health after my Medicare sessions run out?

Yes. Most private health funds include Exercise Physiology under extras cover. Check with your fund to see if you’re eligible for rebates.

Can I have hydrotherapy under Medicare?

Hydrotherapy with your Exercise Physiologist at the Parafield Gardens Swim School may be part of your program. Ask us how it works with your plan.
